Can Modafinil Cause Dehydration?
Modafinil is sometimes associated with symptoms such as dry mouth, raising the question of whether it also causes dehydration. The available human evidence is limited, but studies examining water turnover, sweating, and heat regulation provide some useful clues.
Does Modafinil Actually Dehydrate You?
The most directly relevant evidence comes from Cuddy et al. (2008), who studied 12 male Air Force Special Operations personnel during 72 hours of continuous wakefulness. Participants received either modafinil or placebo, and water turnover was measured using deuterium oxide.
Average daily water turnover was 8.8 ± 1.0 L/day with modafinil and 9.0 ± 1.5 L/day with placebo, with no significant treatment difference.
The study was small and involved physically fit male military personnel under the unusual condition of prolonged sleep deprivation, so the result cannot necessarily be generalized to everyone who takes modafinil.
Can Modafinil Cause Dry Mouth?
Inoue et al. (2021) conducted a randomized, double-blind, placebo-controlled trial in people with idiopathic hypersomnia. Thirty-four participants received 200 mg of modafinil each morning for three weeks.
Dry mouth was reported by 3 of the 34 participants receiving modafinil, or approximately 8.8%. Headache and nausea were also among the more frequently reported adverse events.
The trial recorded dry mouth as an adverse event, but it did not measure body-water loss or establish that participants reporting dry mouth were dehydrated.
Does Modafinil Make You Sweat More?
Launay et al. (2002) studied 10 men who received either 200 mg of modafinil or placebo before resting for 1.5 hours in a climatic chamber at 45°C, with relative humidity below 15%.
Sweat rate was significantly lower with modafinil throughout the heat exposure.
What Happens With Modafinil in Extreme Heat?
Despite the lower sweat rate, rectal temperature increased by 0.50 ± 0.04°C with modafinil compared with 0.24 ± 0.05°C with placebo by the end of the Launay experiment.
Mean skin temperature also increased more with modafinil, and total body heat storage was higher. The researchers concluded that modafinil produced a slight hyperthermic effect during passive exposure to dry heat and could impair heat tolerance.
Reduced sweating can limit heat dissipation, so the higher body temperatures in this experiment are better understood as a thermoregulation finding than evidence of dehydration.
How Can You Tell If You Are Actually Dehydrated?
Hydration can be assessed using measures such as urine osmolality, urine specific gravity, changes in body weight, urine volume, and urine color. Most laboratory measures are not practical for routine use at home.
Kostelnik et al. (2021) reviewed 10 studies evaluating urine color as a hydration marker. Urine color generally correlated with more objective urinary measures of hydration, although the strength of the relationship varied between populations.
Urine color can provide a rough indication of hydration status, but it is not a precise measure of total body water.
Bottom Line
The available human evidence does not support dehydration as an established effect of modafinil. A small controlled study found no increase in water turnover, while separate studies show that dry mouth can occur and that modafinil can alter heat regulation under extreme conditions.
