Support Worker
Community Support
Workers (case managers) assist members in defining their goals for
recovery and in meeting those goals. The strengths of the member
are emphasized in the recovery plan. Community Support Workers first
build a relationship with the member, develop the recovery plan
together, and help the member to implement the plan. Some of the
activities of the community support worker are to assist members
to maintain psychiatric stability; visit members in their homes;
teach menu planning, how to grocery shop, and how to cook; assist
in looking for housing and employment. Community Support Workers
also work with members on substance abuse issues and physical health
care needs.
Job
Responsibilities:
- Engaging
and working with the member in the development and implementation
of a recovery plan.
- Making home
visits to teach skills such as cleaning, budgeting, and laundry.
- Supporting
the member in a crisis situation.
- Assisting
members to obtain entitlements and to locate and use community
resources.
- Locating
appropriate housing.
- Providing
supportive counseling.
- Assisting
members in symptom self-management.
- Providing
education and support to families and serving as a support system,
when appropriate.
- Providing
advocacy for the member in the community.
- Preparing
and submitting required paperwork (daily progress notes, recovery
plan updates, reports) and entering information into the Behavioral
Health Information System.
- Teaching
the importance of and monitoring medication compliance.
- Participating
in the on-call system.
- Working variable
hours between 8:00 a.m. and 9:00 p.m.
Additional
Requirements/Personal Qualities:
- M.S.W. preferred.
B.A/B.S. in relevant human service field or H.S. degree and credentialed
through a combination of education and experience
- Previous
relevant paid mental health experience leading to a working knowledge
of mental illness
- Positive,
energetic, able to engage members
- Flexible,
willing to do what is necessary to fulfill members' needs
- Excellent
organizational and communication skills
- Enjoys working
as part of a team
- Must have
own car. Green Door pays mileage.
- Valid driver's
license and clean driving record
- Must have
a cell phone. Green Door pays part of the cost.
